Customer Order Form

All orders shipped FedEx Air Service, F.O.B. Providence. Shipping costs are prepaid and added to each invoice. Handling charge of $5.00 is added per order. Terms are net 30 days from date of invoice. No other items or conditions of sale binding unless authorized by a Verichem representative. Prices effective January 1, 2025 and subject to change without notice.
